  3. Scope This standard applies to all company sites and those of subsidiary companies of HLL. It applies to all maintenance, construction, demolition as well as non-routine high risk process activities like vessels cleaning and any non-routine activity in a high risk areas like fuel storage area; corrosive chemical storage area. The procedure is applicable to all such tasks apart from the following exceptions:
  4. Scope Simple, low hazard, tasks carried out under the direct supervision of the person who has operating control of the asset. Such activities are specifically exempted in writing by the Unit Head after consultation with safety officer / concerned manager and carrying out a risk analysis study. Maintenance tasks carried out within maintenance off workshops/contractor sheds line, on decontaminated, equipment and materials. Minor adjustments done of packing lines by the operators themselves. Routine operations which have no specific hazards associated with them and have no effect on plant operations, e.g. instrument chart changing
  5. Scope — routine sampling — oiling and greasing activities — cleaning / housekeeping activities in offices and amenities. However it is essential that the persons carrying out these tasks are adequately trained and fully aware of hazards in their operating area. Untrained persons should never be put on such tasks. Routine cleaning activities (office floors, roads, canteen etc.) and gardening activities that are carried out away from foreseeable plant, process, location and equipment hazards, are outside process vessels and bunds, and have been subjected to formal plant work procedures.
  6. DEFINITIONS Permit to Work (PTW): A form, which sets out, agreed work to be done on identified equipment, or in an identified area, and the precautions to be taken. It is a clear record that all foreseeable hazards have been considered in advance and it provides a formal hand over mechanism of equipment/system between the owning group and another group responsible for executing this work. There shall be at least three different types of P TW forms: General Permit Permit for Work on Fragile Roof and at Height Permit for Work in Confined Spaces These are the minimum requirements. A site may use additional P TW form based on specific site hazards: e.g. some of the sites may like to use separate form for hot work or work with hazardous chemicals like anhydrous ammonia. However, all such additional P TW forms shall be aligned to include the details specified in the forms attached to this Standard.
  7. DEFINITIONS Issuing Department: The group which has ownership and/or operating control of the asset to be worked on or area (plant block) to be worked in e.g. Production department for production halls. Issuer: A trained person who is the designated in-charge of the issuing department in the concerned shift and has been authorised in writing to issue P TW e.g. in case of production area it could be the process officer. It should be noted that in any shift only one person should have authority to issue a P TW for any work in a defined block/area.
  8. DEFINITIONS Permit Approval and Authorisation A permit is approved and authorised after ensuring that all possible hazards have been identified, safety precautions have been taken and these have been explained to the persons carrying out the job. In case the issuer is overall in-charge for getting the job done and will hand over the permit directly to the persons carrying out the job, then he approves and also authorises the permit.
  9. DEFINITIONS Acceptor: A person who is the representative of the group responsible for the execution of agreed work and who has been trained to be competent to control the work to be carried out, and authorised in writing. A signature of acceptor is required on the Permit only if 'contract' workmen are carrying out the job.
  10. DEFINITIONS Cross Reference System.' A system for coordinating two or more P T W, which depend on common electrical or process isolations.

  19. GENERAL RULES Only persons who have been trained and authorised shall issue, authorise or accept P T W. Only work, which is specified on the P T W, shall be undertaken. Where preparation for a job includes blanking of pipelines for physical isolation, separate P TW shall be issued for the isolations, main task and reinstatement of pipe work or equipment. A cross reference must be made on the relevant permits. For •obs of long duration, as far as practicable, the P TW shal cover only a particular phase of the task at a time, that can be fully specified and to be completed within the duration mentioned in the Permit. If work requires isolation across plant boundaries, a separate isolation P TW shall be issued by one plant as evidence that the task can proceed on the interconnected system in the other plant.
  20. GENERAL RULES Normally only one P TW shall be issued at any one time for any one item of equipment. However, where it is considered safe to authorise two or more separate jobs being carried out under common electrical or process plant isolations, then the separate PTWs shall be coordinated by a Master Isolation P T W. In this system isolations are carried out through the master P T W. All the other P TW issued under the Master Isolation are referred directly on the master P TW forms, (or on a separate log sheet for more complex situation e.g. annual shutdowns or major equipment overhauling). In such cases equipment covered under P TW shall be energized only when all the refereed Permits are returned. Energisation shall be done only through the master P T W. In a defined area a single P TW book — one each for general work, work at heights and confined space entry shall be in use at any one time. At least 2 copies of a P TW shall be made. The duplicated copy of each Permit will remain in the book, and when all the Permits in a book have been used, the book itself will be retained in a designated location along with the permits returned - for at least 1 month.
  21. OPERATION Issue of Permit to Work — The Issuer shall, along with the Acceptor of the Permit, shall visit the job spot and iointly assess possible hazards. — The issuer shall complete Sections 1 to 10 of the PTW, specifying the task to be done, possible hazards, PPE to be used and attach separate Method Statements if required. This shall be done in consultation with engineering manager/officer (for engineering jobs) and the contractor supervisor (for contractor jobs). — The issuer shall then request required isolation (electrical; services; process) and get these certified on the permit as appropriate. — After filling the Precautions Checklist (section 12) the permit then shall be approved / authorised by the issuer (section 13). In case of engineering jobs the permit shall be authorised jointly by the issuer and the designated engineering officer/manager.
  22. OPERATION Authorisation requirements for Permits for high risk jobs In case of Height and Confined Space work, additional authorisation of Safety Officer is required. In case job involves work on fragile roofs, the concerned Department Head shall authorise the permit. For those Confined Space works that require entry into a vessel which contained toxic/ corrosive or flammable material or if the job requires hot work like welding within the confined space then also the concerned Department Head shall authorise the permit. Attachment of Method Statement/SOP is mandatory in above-mentioned very high hazards jobs (cases stated above where the P TW require authorisation by Department Head). These are the minimum requirements. Based on site's own hazard analysis, more tasks may be covered by additional authorisations as mentioned above.
  23. OPERATION Authorisation requirements for Permits for high risk jobs For major capital equipment installations and construction projects being undertaken within a Fenced-off Strictly Segregated Area, a distinct Project Work Permit system shall be followed in accordance with this Standard by the group responsible for project execution.
  24. OPERATION Authorisation requirements for Permits for high risk jobs The hand over from Issuer / Engineering officer (or manager) to Acceptor must include a visit to the job location. The Acceptor shall signs the section 14 of permit (both the original and the copy in the permit book). The first copy shall be handed over to the Acceptor. However acceptor's signature are mandated only in cases where the job is contracted. The period of validity for a PTW in defined areas within a site shall be the estimated time for the completion of the job, but no more than 8 hours or the period during which the Issuer / Engg Officer is present at site. Work beyond this period shall be re-authorised by the respective Reliever(s) after re- assessment of the job location.
  25. OPERATION Authorisation requirements for Permits for high risk jobs An Acceptor needing to continue with the job into the next shift/period must be asked to contact the Issuing Plant in the next shift and ensure the validity is extended provided that no change has taken place in the conditions stipulated in the permit. The permit issued on a particular day may be extended, if required, only for the shifts on that particular day. For work extending beyond the day and to continue on the next day(s) a fresh permit shall be issued as per the requirements stated in Section 6.3.1.1 to 6.3.1.7 (See Sec. - Operation) above after examining the job location and re assessment of hazards as well as identifying the names of persons who will carry out the job. If the team / contractor carrying out the work has to change or the job is abandoned then the Acceptor will sign off the permit as "Work Suspended" in section 17 of the PTW and hand the permit to the Issuing Plant. The issuing plant shall formally close such permits after examining the job location (A new permit will be required for restarting the job).
  26. Acceptance of Permit to Work In accepting a P TW the Acceptor must — understand the scope of work to be carried out, read the permit and understand the isolations/preparations made — visit the Site with the issuer / engineering officer if necessary — discuss and resolve any outstanding questions — sign the permit and retain the first COPY — acceptor has the responsibility to ensure that all the other people doing the work are familiar with the contents of the Permit to Work, that the specified precautions are observed and that a new Permit to Work is obtained if the nature of the work changes.
  27. During the Task The Issuer covering the plant area should inform the acceptor of relevant changes in plant environment, and co-ordinate other jobs in the area to prevent unsafe interactions. — If the task requirements change, isolation and the specification of the work must be re- examined, and if necessary a new Permit to Work issued.
  28. When the Task is finished — Testing Equipment / Trial Energisation When it is necessary to test equipment without fitting back all the safety devices, permission to reconnect the equipment to its energy source must be obtained from the issuer (and engineering officer as required) who must be party to the testing. It is essential that all personnel are safely clear from the area and that equipment is again isolated after testing.
  29. Work Completed The acceptor (i.e. the contractor supervisor or own employees on the job) must detail on the Permit to Work and describe to the issuer the precise state of the equipment when the task is finished, and confirm the area has been left clean and tidy. He will confirm this by signing the section 16 of the permit.
  30. SYSTEM AUDIT The Site Safety Officer shall conduct formal audit of P TW system covering all defined areas at site, at to confirm its least once in a month, appropriateness and full compliance to all provisions of this Standard. The audit shall be carried out using a checklist developed based on this Standard. Formal audit reports shall be prepared and appropriate corrective actions identified. The Unit Head shall sight audit reports and assign responsibilities for corrective actions. The Unit Head and Department Head shall personally carry out daily random checks of Work permits
